How are VAT and customs fees handled for orders within and outside the EU, UK, and USA?

For orders within the EU, UK, and USA, VAT and customs fees are included in the purchase price (DDP - Delivered Duty Paid). For orders outside these areas, the customer pays VAT and customs fees separately (DDU - Delivered Duty Unpaid).
